Personal Care Services

Personal Care Workers provide non-medical services to people with disabilities, the elderly or patients suffering from mental disorders. They help with day-to-day activities such as:

  • Serving as a companion to clients

  • Transporting clients to and from appointments, errands, activities, etc.

  • Managing medication

  • Preparing meals

  • Interpersonal skills, including exceptional patience and listening skills

  • The ability to provide care across a variety of settings, including group homes and day service programs

  • Performing light housekeeping duties (e.g., keeping room clean, laundry)

  • Providing bathing and dressing assistance

  • Keeping proper care records and reporting changes to nursing team

  • Encouraging socialization and participation in community activities

  • Following patient care plan

  • Communicating with families, co-workers and supervisors regarding care

  • Assisting with transfer and mobility

  • Shopping and running errands

  • Maintaining a safe and comfortable home environment

Therapy Services

Therapy services are performed by a licensed Physical Therapist, Occupational Therapist or Speech-Language Pathologist.

  • Physical Therapy - a set of rehabilitative procedures that include specific exercises and movements to help patients improve their physical function or regain mobility. These professionals usually work with patients that have experienced an injury or illness, or those recovering from surgery.

  • Occupational Therapy - involves the assessment and treatment of patients in order to help them regain or maintain mobility and function. The techniques employed by these professionals enable patients to regain the ability to perform day-to-day activities or return to work sooner.

  • Speech Therapy - assess, diagnose, treat and help prevent communication and swallowing disorders. Speech-language therapy teaches the tools, resources and support necessary for developmental growth that will help patients develop and maintain the ability to speak more clearly, understand and express thoughts and feelings, and eat and swallow safely.

Nursing Services

Nursing services are carried out by a Registered Nurse, Licensed Practical Nurse or Certified Nurse Assistant.

  • Registered Nurse - completes nursing assessments to determine the level of care you need, and then communicates the information with your physician and other care providers to establish your plan of care and provides medical skills.

  • Licensed Practical Nurse - works closely with an RN to carry out your plan of care and provide skills such as:

    • Nurse case management encompasses patient advocacy and interdisciplinary communications to make sure you are receiving the care you want and need.

    • Patient/Family teaching on health conditions, medications, how to manage at home and more.

    • Medication Management helps you set up medications, teaches you about your medications and helps or administers medications to you.

    • Injections, IVs and Lab Work can be done in your home according to your physician’s orders.

    • Wound Care can also be performed in your home depending upon the severity and level of care required.

    • Catheter Care includes preventing infection, replacement and supply management.

    • Ostomy Care teaches you and your family how to care for your specific type in order to prevent infection, reduce skin breakdown and manage it daily.

    • Ventilator & Tracheostomy Care can be taught to be managed by family members, however, this level of care usually requires full time skilled nursing care.

    • Respite Care is done by a nurse, certified nurse assistant or personal care assistant depending on the level of care required.

  • Certified Nurse Assistant (CNA) or Home Health Aid (HHA) are licensed by the state to help you with basic medical care and activities of daily living that may include:

    • Vital Signs: heart rate, respirations and blood pressure. CNAs monitor your vital signs to make sure they are within normal range for you and report any changes to the nurse.

    • Monitors your health condition and reports any changes to the nurse to make sure you get any additional care you need.

    • Transfers: help you move to and from your bed, chair, shower, ect

    • Personal Hygiene Care: bathing, oral care, perineal care, shaving, ect

    • Respite Care to give families and other caregivers a break (based on level of care)

Medical Social Worker Services

Medical Social Workers performs various duties to support the health of patients and their family members. Their responsibilities vary based on their company and specialty, but some of their most common duties include:

  • Meet with patients and family members to discuss needs, conditions, resources and options

  • Work with patients to set goals, form plans to reach those goals and monitor progress

  • Coordinate care plans and support for patients who require multiple services at a time

  • Provide informal counseling and advice on how to live with illness or injuries

  • Address any issues that may arise in meetings or counseling and therapy sessions

  • Meet with new patients to discover which services can properly address their special needs or restrictions

  • Assist with patient discharging from a health care facility and returning home

  • Perform research into various medical subjects and stay up-to-date on industry news
