Therapy Services

Therapy services are performed by a licensed Physical Therapist, Occupational Therapist or Speech-Language Pathologist.

  • Physical Therapy - Physical therapists work with clients who have experienced a decline in functional movement. They work with clients to create a personalized set of exercises and movements. The goal of these exercises is to help clients with pain relief, mobility improvement, functional recovery, and symptom management for chronic disease or acute conductions. 

  • Occupational Therapy - Occupational therapists help clients who have experienced a loss of motor function learn self-care skills, overcome physical challenges, and manage emotional and social issues. Through education and the use of adaptive tools, they teach clients how complete household tasks and personal care like bathing and grooming. Occupational therapists also work with clients to create a safer home environment, limiting the risks of falls and injuries.

  • Speech Therapy - The delivery of speech-language services in the home allows for therapy activities to be tailored to address "real life" functional needs. These therapists work with clients to assess, diagnose, treat and help prevent communication and swallowing disorders. The goal of this type of therapy is to help clients develop and maintain safe swallowing techniques, speak clearly, and be able to understand or express thoughts and feelings.

Nursing Case Managment

Case management is a collaborative process in which a case manager works with clients to ensure they obtain the best quality of care. Having personalized care management has many benefits, including decreased readmission rates, increased efficiency in care delivery, improved patient satisfaction ratings, and reduced mortality rates.

We Care Home Health recognizes the importance of case management and is dedicated to providing individualized care. Each of our extended care clients work with a case manager to establish a plan of care that is idealistic and works towards personal care goals. By working closely with clients and families, we hope to provide support that brings peace of mind and relives stress.

  • Examples of Nursing Case Management

    • Scheduling for extended care cases.

    • Managing supplies.

    • Medical appointment reminders and scheduling.

    • Managment of orientation and trainings.

Nursing Services

Nursing services are carried out by a Registered Nurse, Licensed Practical Nurse or Certified Nurse Assistant.

  • Registered Nurse - RNs play a vital role in managing medical cases that have a higher complexity and need for critical thinking. They complete nursing assessments to determine the level of care needed and communicates with the clients’ physician or other care providers to establish a plan of care.

  • Licensed Practical Nurse - LPNs work closely working under the supervision of either a doctor or a registered nurse. They are trained to provide nursing care that coincides with the plan of care set by the physician or supervising RN.

  • Examples of Nursing Skills Provided

  • Certified Nurse Assistant (CNA) or Home Health Aid (HHA) CNAs are licensed by the state to provide basic medical care. They work closely with an RN or LPN to care for clients and help with activities of daily living that may include:

    • Reporting Changes in Base Line to RN

    • Vital Signs: heart rate, respirations and blood pressure. CNAs monitor your vital signs to make sure they are within normal range for you and report any changes to the nurse.

    • Monitors your health condition and reports any changes to the nurse to make sure you get any additional care you need.

    • Transfers: help you move to and from your bed, chair, shower, ect

    • Personal Hygiene Care: bathing, oral care, perineal care, shaving, ect

    • Respite Care to give families and other caregivers a break (based on level of care)

  • Nurses teach about diagnosed health conditions, disease development, and how to manage disease symptoms.

  • Nurse help set up medications, teaches clients about their medications and helps or administers medications to clients.

  • Nurses can administer injections or IV medications according to physician’s orders in the comfort of the client’s home.

  • Nurses can perform wound care in a client’s home depending upon the severity and level of care required.

  • Nurses teach clients and their family how to care for their ostomy, prevent infection, and skin breakdown.

  • Nurses can clean skin around PICC lines and change the bandages according to the physician orders.

  • Nurses educate clients on proper airway care. They also maintain the ventilators, change tracheal tubes, clear mucus from the lungs.

  • Nurses educate to prevent infection, skin deuteriation and can replace G-Tubes according to physician orders.

  • Nurses educate to prevent infection and can replace catheters according to physician orders.
